When visiting our Urologists – What should I bring?

Please bring your insurance card(s), photo I.D. and a list of your current medications with you to your first visit. We recommend that you arrive to your appointment at least 30 minutes prior to complete any patient registration and health history forms. Please enter our Patient Portal to fill out new patient paperwork.
